Requesting A Sacramental Record

To obtain a copy of your certificate of Baptism, Marriage, First Holy Communion and/or Confirmation, please complete the request form here.

A donation of $15.00 is requested for per sacrament record.  Donations can be made by credit card via WeShare.

If you would like to pay by check, please write a request with as much information regarding the sacrament as possible: date, year, date of birth, parents’ names, sponsors/witnesses, the address where the certificate should be sent, a $15 check for should be made to Holy Innocents Church.
